What is this "Use of uninitialized value..."

What is this and how do I stop it. It does not affect the program,
everything runs just fine it is just a warning but I would like it to
stop.

Every time I create a new form another line shows up. On one program I
have it has a few dialogs for people to enter data and so I get the same
line over and over again.
