Yes. We mostly use etherpad for this type of thing (collaborative editing like 
Google docs, but free software).

https://promo.notes.kde.jefferai.org/ (I made you an account, you'll get 
details by email). When you log in you can start a new pad with potential 
survey questions, if you like :-)

So the main thing is what we want to ask, why and how... Also what we're going 
to do with this data when we get it (which shapes the what, why and how).
