On some distros, on some versions of kmail, the standard Forward 
template may be:

%BLANK

You need to replace that with, as a minimum:

%TEXT

You can insert additional text and commands if you choose. For 
example, you could set the template to (what I think is the 
"default"):

----------  Forwarded Message  ----------

Subject: %OFULLSUBJECT
Date: %ODATE
From: %OFROMADDR
To: %OTOADDR

%TEXT
-------------------------------------------------------

Experiment to find the result that you want.
