I re-read this advice, and re-checked my special collections. I had 
previously missed two trash folders, one called 'Wastebin' and one called 
'Trash'. Cleaning up the latter has caused kmail to spring back into 
life and cpu usage to drop.

Is there a bug for this? It's such an odd problem.
