It needs to be another KDE program so we can check if it is something KMail 
does wrong or if the setting used by the core infrastructure is.

E.g.

kioclient exec /path/to/some/pdf

Given our output of the ktraderclient command I'd say the latter.

If you run
kcmshell4 filetypes

does it list the program you want for application/pdf?
Is it the top of the list?
