Just tried that with my KMail 5.10.3 on KDE Neon. Behaves exactly as you 
described.

I think adding Reply-to automatically makes sense, and implementing it the 
same way as To, CC, and BCC too. Just the focus after opening the composer 
should be in the first To text box.
