I finally figured it out - I experienced the SAME issue 4 years ago. When adding headers using http_xproc() procedure, you MUST ensure you have a CRLF at the end of each header. I was missing one on the last header I added. So I'm guessing the server just sits there waiting for the next header until it times out. (you can't see this in the debug log) Apologize for wasting your time.
