[dylan] Dylan Programming Guide revived

As most of you know, I'd done much of the documentation conversion a year
ago into the new formats. I hadn't done the bulk of the work on the Dylan
Programming Guide book though ...

This week, that's changed!

I'm still working on fixing up links and images ... but the bulk of the
re-formatting and cleanup has been done. It took 3 really boring days this
week ...

This also lets us produce a pretty nice looking PDF (doing so requires
changes that aren't complete / pushed yet).

For now, this is more of an FYI ... within a couple of days, I'm hoping to
have it at a point where others can take a look for things that I've missed.

Hopefully this can be wrapped up in time for our 2013.1 release in about a
month!
