updated manual

Ahoy,

There is an updated Distel user manual at
http://www.bluetail.com/~luke/misc/erlang/distel-user.ps.gz

The main changes from the previous edition are that it covers the
Refactoring command, and includes a pretty picture to illustrate the
debugger states.

There is also an INSTALL.WIN32 file in CVS, documenting the
not-terribly-smooth installation procedure on windows.

I'm planning to do a new release this week, so please give feedback.
