I am now having this exact same problem.  I just recently upgraded my package
to DBD-mysql41-3.0006_01  from DBD-mysql-3.0002_02 and now I get the 
following error:

prepare_cached(SELECT s, l
FROM   TEST
WHERE  l = ? AND s = ?
) statement handle DBIx::ContextualFetch::st=HASH(0x85d9b60) still Active 
at /home/y/lib/perl5/site_perl/5.6.1/Ima/DBI.pm line 391

Does anyone have a fix for this?  And downgrading the package is not a 
viable solution :)
