So far, I have found only one way to do this, and it ain't pretty.
Create a new PDF and for each page: create a new page and write the
watermark to it, then copy the page from the old PDF. This places the
watermark under the page contents.
See the perldocs for:
PDF::API2::importPageIntoForm -- to copy the old page
PDF::API2::Content::formimage -- to place it in the new PDF
