I have never seen such a thing in my entire life. Every double acrostic puzzle I have ever seen has had a one-to-one correspondence between answer letters and grid letters, and a reverse index from the grid back to the answers. All the double acrostic puzzles in _Games_ magazine and in the New York Times Magazine section have this form, for example. I would be interested to see a counterexample.
