with phpLDAPadmin 0.9.8.5 (downloaded from sf.net) i can
create a user WITHOUT an error (-:

Hi,

you can reproduce this error very easly installing OpenDS LDAP_Server
(opends.org) 
within 5 Minutes and installing phpldapadmin 1.1.0.5-6 within an debian
lenny.
Now create a "User account" with phpldapadmin - and you will get the
error.

Can i help you to track down this prob ?
