Initial Comment:
When attempting to create a group with the Posix Group template, the form that is produced does not allow you to enter a GID Number.  GID Number is a required field, hence you cannot actually add a group.  The other two fields on the form are editable (Group and Users), but not GID Number.  The same issue is described here: http://www.nabble.com/phpLDAPadmin-Group-object-creation-td12086189.html, but this code fix is already present in the version I am using (1.1.0.7).

LDAP server version: Red Hat Directory Server 8.0.4
Web server: Stock RHEL5 Apache/PHP (2.2.3/5.1.6)
OS: RHEL5.3 x86_64

Message:
You have two options here. Change the template so that PickList is not
used, or create a posixGroup entry.

The next release of PLA will display a warning when a pre-populated field
that is autogenerated is empty (and stopping the the form from being
submitted).
