Initial Comment:
When trying phpldapadmin with a brand new uninitialized slapd server, the following message appears in the ldap tree after logging in:

This base entry does not exist.Create it?

However, the "Create it" link does not work. because the appropriate form that the javascript tries to submit is not in the HTML created.

I attempted to fix this, as I have worked on phpldapadmin before, but it's a bit of a mess and I don't have a lot of time to invest in fixing it right now.
