Summary: "Create one?" button not working

Initial Comment:
Brand new install of phpLDAPadmin 1.2.0.5 with new OpenLDAP server. I log in with the web interface and see this: "This base entry does not exist.Create it?" I click on Create it and nothing happens, and I receive a javascript error in Firebug.
