Initial Comment:
I have just installed phpLDAPAdmin and when I login, there's a message on the left hand side saying:

dc=my-domain, dc=com
This base entry does not exist. Create it?

"Create it" is linked to "javascript:document.create_base_form_1.submit()". When I click on "Create it", nothing happens - however an entry appears in the JavaScript console:

Uncaught TypeError: Cannot call method 'submit' of undefined

The error occurs in IE8, Firefox 3.6.3, and Google Chrome Beta 5.0.375.99.

LDAP server: localhost
Web server: localhost
php 5.2.13
Installed on a QNAP NAS TS-239 Pro, firmware 3.3.0 Build 0629T using their OpenLDAP package: http://forum.qnap.com/viewtopic.php?f=211&t=31322
