Summary: Can't hide required attributes

Initial Comment:
When a schema-required attribute is defined as hidden on a custom creation template, it will show up on the page anyway, even if it has a <value> or <default> setting (or if it is being automatically filled). This does not happen with non required attributes (these are indeed hideable).

phpLDAPadmin 1.2.0.5
OpenLDAP slapd 2.4.19
Apache 2.2.8
PHP 5.2.4
Ubuntu Hardy 8.04

Im attaching my custom template. In this case, the sambaLMPassword, sambaNTPassword, cleartextPassword and sambaAcctFlags attributes are hidden, whereas the uid, uidNumber, gidNumber amd homeDirectory are visible.
