Downloaded version 1.2.2 from Sourceforge after reading Debian Security Advisory DSA-2333-1.
Copied configuration from 1.2.1.1 installation.
Browsed to root:

phpLDAPadmin 1.2.2
OpenLDAP server (slapd) 2.4.11
Apache HTTP Server (httpd) 2.2.9
PHP 5.2.17
Debian GNU/Linux 5.0.9 (lenny) with additional packages from dotdeb.org

Check the permissions of the /etc/phpldapadmin/config.php - setting the
group to "www-data" cleared this error for me...

Cleared cookies, server system rebooted but nothing

$_SESSION should always exist : that sure but ....
$_SESSION[APPCONFIG] not exist

change with
                if (isset($_SESSION[APPCONFIG]))
                        if
($_SESSION[APPCONFIG]->getValue('appearance','minimalMode')) {
                        $display = array(
                                'HEAD'=>false,
                                'CONTROL'=>false,
                                'TREE'=>true,
                                'FOOT'=>false
                        );
                }

work better on other error come from the logo 
in phpldapadmin/lib/functions.php :

 return isset($_SESSION) && !
$_SESSION[APPCONFIG]->getValue('appearance','remoteurls') ? '' :
sprintf('%s://sflogo.sourceforge.net/sflogo.php?group_id=%s&type=10',$proto,$group_id);

i have change to :

if (!isset($_SESSION[APPCONFIG])) return

I am also seeing this error.
Server is a Gentoo install, so not Debian specific. 

Verified via IE and Chrome.

>From what I know of PHP, $_SESSION should always exist. If it doesn't it
has been explicitly unset somewhere.

cache.
