[boost-docs] Query regarding boostbook and documentation

The boostbook and associated pages are lacking in crucial information,
can anyone please point me to:
* a boostbook DTD
* an example of boostbook xml & xsl so I can figure out what content is 
meant to be auto-generated and what is not
* updated instructions for building quickbook, as existing instructions 
for building both quickbook and bjam (now b2) are out-of-date
