The way we do it in PangoCairo is another. The base problem is that FT_Face is not threadsafe. The workaround is to never use it from multiple threads at the same time. cairo-ft facilitates this by having a lock/unlock pair for the FT_Face. PangoCairo does the same. If the user uses the FT_Face by themselves, and creates a cairo_font_face_t for that FT_Face, and the user code does not use the cairo lock/unlock pair, problems will happen. But there really isn't a way to work around that.
