Creating a table of contents

Hi,

Using Cairo with PDF surface I write (create) a multi-page document. During
the process I gather information for a table of contents (basically titles
and page numbers).

When the document contents is complete, I can create the table of contents
and use hyperlinks for the toc entries. So far, so good.

But I want the table of contents to precede the other contents. Is there a
way to achieve this?
