Cheers for the suggestion. I've tried the latest CCID drivers (1.4.16) and don't appear to be experiencing the same stability issues. Am now up-to 30+ plug/unplugs without any USB errors... 

Now to try with a few more readers...

If you use a proprietary driver you should talk to the vendor.

The reader should be well supported by the free software CCID driver, so better try that one instead:
