[PATCH] context switch callback enhancement for state_threads.

We have an app here at Yahoo and we need to know when threads switch
to each other.  I have put together a patch against 1.5.1 here:

It allows one to set callbacks for when a thread is switched in and
out under st threads.

Documentation has also been updated to reflect the new functions.

Right now we're using it for profiling, but there's a lot of
other potential uses for using this code to extend st_threads.

I'd like to keep it under the "normal" build as runtime enabling
of these callbacks can be effecient as there's only a single pointer
check when they are not enabled.
