Implementation

As those following -commits@ may have noticed, I've started working on 
the implementation of the NT port.  The library now builds and exports 
all functions, however some of them are either unimplimented and return 
REC_NOT_IMPLIMENTED, or simply fail, hang or otherwise misbehave.  What 
is currently fatal is the page manager which exhausts memory, I'll look 
at it tomorrow.

The documentation is now somewhat complete.  Everything related to 
thread locking is now documented (which is four functions, one structure 
and two helper classes).  Only thread locking functions are currently 
not available, which will be fixed as soon as possible.
