Sorry, your reports are confusing.

What I'm missing from your reports is the qmail-send logs.
qconfirm-check logs all its actions to the qmail log.  If you don't have
any qconfirm-check log entries in your qmail logs, you most probably
didn't edit the correct dot-qmail file.  If you have qconfirm-check log
entries, they tell you what's going on, just like the usual qmail-send
logs do for each message queued on your system.
