qconfirm not sending challenge

I installed qconfirm.0.14.1 the other day & have run into a small snag; for some unknown reason the challenge message isn't sent. I checked all the logs I could think of and no errors are flagged.

It appears that all other aspects are working correctly. If I a reply to jim_test-qconfirm-8ed445b6538231208ff6763784e9bc18@mydomain dot com (as per the .qmail-jim_test-qconfirm-8ed445b6538231208ff6763784e9bc18 file) everything works fine.

any suggestions what might be causing the problem? need more info?
