My personal preference would be that if full name is empty and nickname is NOT empty then nickname would be used. If both are empty then use first + middle + last. I don't know much about this mms_image directory. I assume it is used when someone sends you a msg with a picture. You say they are jpgs and I guess you have figured out how to write the extra headers they have on the phone. Is there an image count limitation?
