Did you send SI or SL first, and get this one only when the phone tried
to do
a fetch ?

If phone is trying to do a fetch after a SI or SL, "faulty headers" may
be
specific push headers. Can you add msg_dump in production environment?
(Of
course you should test a newer libxml version first.)
