I beleieve this can happen when kannel sends the second PDU before the
SMSC returns its acknowledge - this is fine by the SMPP specs but maybe
some SMSCs dont like this... 

Try to change SMPP_MAX_PENDING_SUBMITS from 10 to 1 in

gw/smsc/smsc_smpp.c:53
#define SMPP_MAX_PENDING_SUBMITS    10

and see what happens.
