I think this and David's reply answer most of your questions, but
I'll add some comments.

These are only going to be useful in FF3 if they have Unicode
character maps.

Now that we have Unicode support for mathematical characters and
Unicode fonts are becoming more common, this is improving.

However, there are never going to be separate Unicode assignments
for different sizes of radical glyphs.

Web fonts (which are automatically downloaded [and cached] when
required) are something that may be useful here.  Web font support
is expected for Firefox 3.1.  See this bug and its dependencies

However, to take advantage of this, the special handling of MathML
stretchy characters needs modification, which could be a future
project.
