Whenever this has come up, I believe that the WG has been consistent in
saying that the correct markup for primes is
<msup><mi>x</mi><mo>&prime;</mo></msup>
where &prime; is  U+2032.
I think it would be good if systems allowed the ascii ' to also work
(just as they make ascii - make the same thing as &minus, U+2212.

This despite the fact that fonts other than TeX ones probably have the
prime raised already as the glyph, the renderer is supposed to just make
the adjustment.

yes I suppose we could put something in chapter 7 (what has always
previously been chapter 6)
