Depending on your distribution of Perl, there might simply be binaries missing to actually load MySQL-drivers. As you are on Windows, I suggest using ProcMon to see if DLLs are searched for and not found. https://docs.microsoft.com/en-us/sysinternals/downloads/procmon > Another question: Does the MySql bin folder need to be in the machine path? Depends on if it contains necessary DLLs. Additionally keep in mind that if adding that dir to PATH works on the shell, you might need to additionally do it for the web server as well.
