Duration:        Permanent-Full Time
Rate:              $50,000-$60,000 /yr
 
 
Job Description
The responsibilities for these positions are:
 
Identifying high risk, high cost clients through the pre-certification
process and/or other methods and determining eligibility. Assuming
ongoing primary responsibility for a discrete caseload of patients.
Assessing the patient's needs by review of medical records and contact
with proper resources.
 
Planning/coordinating services deemed appropriate and cost effective
in conjunction with the medical regimen. Negotiates reimbursement
rates with non-participating facilities/vendors/providers. Providing
ongoing monitoring, evaluation, support and guidance in coordinating
the patient's care.
 
Documenting all pertinent information pertaining to patient progress
and management of care. Submitting periodic written reports, as
required, with cost analyses.
 
 
Requirements
The qualifications for these positions are:
. Minimum of 3 years of pertinent clinical experience required. 
. Minimum of 3 years of managed care experience preferred.
. Current license to practice nursing in New York State required.
. Bachelor's degree preferred.
. Experience in discharge planning and/or community health desirable.
. Excellent verbal and written communication skills required.
. Professional manner that will project a positive image to the
company.
. Certification in case management strongly preferred.
. Computer proficiency required (Word/Excel).
 
The workweek is Monday through Friday, 9:00 a.m. to 5:00 p.m.
