Exchanging homes

My mom who is in her 70s owns a house and I own a condo, both in
northern NJ.  We plan on moving into her house and vice versa.  What
is the best way to handle the move to minimize taxes?  We need
documentation because we have a child that needs to be enrolled in
school in my mom's home.
