Sounds like that video has some problems... since mplayer is doing the
playback, what it says, it what is should be.

For one the offset should be 16 and not 12, so that should help some.

No idea, would need to see debug info, it is possible that there are no
xv ports available on the second run, just to many things to guess at
with out seeing a log.

No, the scaling of the window size is what controls that.
