Non-Title PGCs

Looking over some DVD-Video info at the dvd-replica.com site, I 
discovered that a title in a titleset can consist of more than one PGC.
However, dvdauthor was assuming that every PGC in a titleset was a new 
title.

I have added a new feature where you can now specify, e.g.

     <titles>
         <pgc ...>
             ...
         </pgc>
         <pgc entry="notitle">
             ...
         </pgc>
     </titles>

and a PGC with “entry="notitle"” will be considered to be part of the 
same title as the previous PGC.

dvdunauthor has also been updated to decompile this information 
appropriately.

I haven’t updated the man page yet; I’ll do that soon, I promise. :)
