Be aware that menus are handled in a non-logical way on 4:3 tv and in 16:9 tv.
Maybe in one of these cases your menu are showed the right way, is this the case?

It seems you're facing the same trouble I had a couple of years ago, and the solution is to mux 
several (up to three) menu into the menu clip, with settings for choose among them. The dvd player 
will pick the right one.

Also, if you want to play an intro clip just once, just put it into vmgm and call it "menu" into the 
xml config file (it won't hurt), I usually choose this way.
Otherwise, you can put this clip into the <pre> section of your root menu, controlled via a flag.
You set the flag to `1' into vmgm, then check for this flag to be `1' in <pre> section. If it is, 
then put it to zero and play the clip. But I never tried, this, the other way is much more simple.
