Two more questions for the MKV spec:


   - Is the table in Chapter Specifications
   <http://matroska.org/technical/specs/chapters/index.html> duplicated
   twice?
   - Should the spec perhaps require EBMLMaxSizeLength to be exactly 8?
   Letting it be < 8 doesn't seem like a useful feature, but perhaps I'm
   missing some useful use case.
