Save HTTP Stream related patches

I made these changes patches against XMMS 1.2.7 code...

The patches will allow the user to save http streams to disk based
on the title of the song, as opposed to the title of server (default 
behavior).  So that filename are like Band-Song.mp3 instead of 
SomeServer.mp3 (although it falls back to SomeServer.mp3 if needed).

Also, the patches create a toggle item on the main general menu for 
faster toggling of "Save to disk."  The patches also set an option on 
the mpg123 configuration dialog (Streaming tab) that allows the user 
to select whether to append files instead of the default write.

I mainly wrote put these in for my own use, but maybe they're actually
useful enough for others to use...

As far as I'm aware there's only one minor bug.  If multiple sites are
in the playlist, and you try playing by clicking a site on the playlist 
it won't save the stream when it plays. In order to be saved, the stream
has to be played either through the play button or the "Save MP3 Stream"
menu item (that I created).
