Originally you mentioned that it doesn't work for the first session, it's 
worst than that. It seems you have to exit and restart if you want a state 
change in the checkbox to take effect. You prolly need a callback of some 
such to reflect in the cfg.append_to_playlist variable the current state 
of the checkbox.
