Filter by form confirmation gone???

OOo Base 125 on WinXP and MySQL

I had created an editing form which I was using with Filter by Form. When
the FBF icon is clicked next to the form were two "Confimation" option
buttons one of which needs to be clicked once a filter criteria is entered
in a field in the form.

These buttons disappeared and I can't get them back. I have rebooted and I
have repaired the OOo installation, but nothing works.

The status is now that I can display the Form Navigation toolbar, click on
the FBF button, enter the filter criteria, but then it stops, I can't
activate the filter.

Help please.
