Nobody answered my question, and I've given up on finding an answer myself:
it looks like there's simply no way to do this in the current OpenOffice,
which basically makes the "Alphabetic Index" feature in OpenOffice useless
(or at least very ugly) for Hebrew.

So I've opened a bug on the OpenOffice bug tracker:

If somebody is tracking, voting on, or (may I dare hope?) fixing Hebrew-
related bugs, please add this to your list.
