If it doesn't print how it should, what exactly does it do? No-one can 
help you unless you give the obvious information needed to provide help.

One obvious one - use A4 paper to test, not real envelopes. That can 
help better show mis-positioning/mis-orientation. It's also cheaper :-)
Also, after you've inserted the envelope into Writer, check by hand the 
page format applied looks plausible. Life is easier if the envelope is a 
separate document, btw.
Otherwise no, I can't add much else - the only problems I've had were 
down to my own failures in understanding the dialogue box.
