[PATCH 0/4] Fixes for multistart

The first of these patches is a revised version of the one I posted in
https://lists.gnu.org/r/bison-patches/2020-11/msg00033.html.  We
rather refer to the "initial rule(s)" than the "accepting rule(s)".
