Taking a look at support/arguments.hpp, it looks like the tree isn't being
flattened before the placeholders are evaluated and the static index range
kicks in.

rule<vector3<int, int, int>(void)> r = int_ >> int_ > int_ compiles, which
leads me to think that this might be intentional, or possibly an oversight. It
might be a bug introduced by the new has_semantic_actions<> CP - I'll look into
that.

Joel, Hartmut, can you guys shed some light on this? Is this a bug, or is this
desired behavior?
