Hope I'm not beating a dead horse. Here's a demonstration of a clean, 
pdf made from the a Drupal collaborative book text.

1) Modified book.module to eliminate the eliminate the class, name and 
id tags in <h1>, <h2> and <h3> for the print-friendly version.

2) Saved the print-friendly version available from the link on this page 
as html source: http://cyberdash.net/celfa03/node/view/2.

3) Open the text in OpenOffice.

4) Copied the page from the webwriter part of open office and pasted it 
into the regular word processing window.

5) Spent 20 minutes modifying the word processing style sheets and 
making a few layout adjustments (Note that I'm a little slow with this 
since I don't usually do anything but straight text in OpenOffice).

6) Exported to pdf: http://cyberdash.com/temp/calendar.pdf

BTW: Note that the links don't work, but that's because I'm still 
unfamiliar with the pdf function in openoffice.
