Changing &#38; to &amp; (and &#61; to =)...

results in:

<a href="http://www.domain.com/index.php?A=1">LINK</a>
<a href="http://&">LINK</a>
<a href="http://B=2">LINK</a>
<a href="http://&">LINK</a>
<a href="http://C=3">LINK</a>
