Please let me know what I can do to help this move forward, as without this
patch eqn is completely unusable for producing web feeds. Outputting entity
references without an accompanied entity declarations is a bug IMHO.
The approx issue on the other hand is of a much lower priority to me as a user
as it (with the patch applied) could be easily worked around with \[u2248].
