Hi Branden,

I started on this yesterday, so rather than both of us committing the same 
thing I've attached my patch, use it if you want. I think the patch in  
comment #4 for the footnote double counting with tbl is probably still
viable.

Oh, thank you for the improvements to my documentation, I approve. (The reason

I chose Groff-PDF... rather than groff-pdf... is so I could type "oku{tab} 
doc/G{tab}", but laziness is probably not a good reason for breaking 
convention!!!)
